BPH vs Prostate Cancer: Key Differences to Know

Quick answer

BPH vs prostate cancer comes down to this: BPH is a non-cancerous enlargement that squeezes the urethra, while prostate cancer is a growth of abnormal cells. BPH does not turn into cancer or raise its risk, yet symptoms can overlap and both can exist together. Only a doctor's exam and tests can tell them apart.

What is the difference between BPH and prostate cancer?

BPH is a non-cancerous enlargement of the prostate. Prostate cancer is a growth of abnormal cells that can spread. They are different conditions.

NIDDK says BPH is not caused by cancer (NIDDK, Enlarged Prostate (BPH)). The NHS adds that BPH does not raise the risk of prostate cancer (NHS, Prostate Enlargement). A man can have one, the other, or both.

Do BPH and prostate cancer cause the same symptoms?

They can, which is the reason symptoms alone never settle the question. A weak stream, urgency and night trips fit BPH, but early cancer often causes no symptoms at all.

Because of this overlap, new urinary changes need an exam. Do not wait to see if they pass, and do not self-diagnose from a list.

  • Symptoms alone cannot rule cancer in or out.
  • A normal-feeling stream does not rule cancer out.
  • Having BPH does not protect you from cancer.
  • Pain, blood in urine or unexplained weight loss should be reported quickly.
Same-day care

Cannot pass urine at all: emergency room. Blood in urine, fever or chills with painful urination, or severe belly, back or side pain: same-day care.

How do doctors tell BPH from prostate cancer?

Doctors use a symptom history, an exam, urine tests and often a PSA blood test, then decide whether more tests are needed.

PSA is a protein made by normal and cancerous prostate cells. BPH, infection and a recent biopsy can also raise it (NCI, PSA Test). About 25% of men biopsied for a high PSA turn out to have cancer, so PSA is a prompt for talking, not a verdict (NCI, PSA Test).

Screening is a personal decision made with your doctor. The USPSTF advice for men ages 55 to 69 is to decide together after weighing benefits and harms (CDC, Prostate Cancer Screening).

  1. Write down symptoms

Include when they began and how often they happen.

  2. Ask about PSA

Ask whether screening makes sense for your age and family history.

  3. Follow the plan

Keep taking prescribed medicine and go to each follow-up.

Can a supplement like Prostadine rule out cancer?

No. A supplement cannot diagnose, treat or rule out BPH or prostate cancer, and Prostadine makes no such promise.

The pomegranate studies we found enrolled men who already had prostate cancer. Neither had a placebo group, neither studied BPH, and neither tested Prostadine (Pantuck et al., Clin Cancer Res 2006) (Paller et al., Prostate Cancer Prostatic Dis 2013). The sales page says its ingredients are "clinically proven to support the health of your prostate and urinary system". No trial of Prostadine exists to back that wording.

Tell your doctor about every supplement before tests, since some products can interact with medicines. Our guide on supplements and prostate medicines covers what to ask. Start with the prostate basics guide if you are new to this.

FAQ01

Quick answers on BPH and prostate cancer

Can BPH turn into prostate cancer?

No. BPH does not become cancer and does not raise the risk of it (NHS, Prostate Enlargement). The two are separate conditions that can occur in the same man, which is why new symptoms should still be checked by a doctor.

Does a high PSA mean I have cancer?

Not by itself. A level above 4.0 ng/mL is generally called abnormal, but BPH, infection and exercise can raise it too (NCI, PSA Test). Your doctor weighs the number with your age, exam and history before suggesting next steps.

Which doctor checks for both conditions?

A primary care doctor can start the work-up, and a urologist is the specialist for the prostate. Ask for a referral if symptoms persist, if PSA is high, or if your family has a history of prostate cancer.
