What did the pomegranate juice prostate study find?
The study found that PSA doubled more slowly in men who drank pomegranate juice daily. Mean PSA doubling time rose from 15 months to 54 months (Pantuck et al., Clin Cancer Res 2006).
The men already had prostate cancer. Their PSA was rising after surgery or radiation. Each drank 8 ounces of juice a day, and no serious adverse events were reported.
The authors said the results "warrant further testing" in a placebo-controlled study. That is a call for more research, not a finding that the juice works.

What does PSA doubling time mean?
PSA doubling time is how long it takes a man's PSA number to double. A longer time means a slower rise.
PSA is a protein made by normal and cancerous prostate cells. BPH, prostatitis, infection and even a recent biopsy can raise it, and finasteride and dutasteride lower it (NCI, PSA Test). A PSA number alone does not show what is going on.
A slower PSA rise is not the same as healthier tissue or fewer symptoms. The studies did not measure urine flow, night trips or bladder control.
BPH is a non-cancerous enlargement that affects many older men (NIDDK, Enlarged Prostate (BPH)). These studies were done in men with prostate cancer, so they say nothing about BPH.
Why can't this study prove pomegranate helps?
It cannot prove it because no placebo group was used. Without a comparison, we cannot tell how much of the change came from the juice.
The extract trial had a similar limit. It gave pomegranate extract at two levels for up to 18 months with no placebo arm, and 42% of men stopped before the end, mainly because PSA kept rising. The authors called the meaning of the slower doubling unclear and asked for a placebo-controlled study (Paller et al., Prostate Cancer Prostatic Dis 2013).
