PSA Test Explained: What the Number Can and Cannot Say

Quick answer

PSA test explained: it is a blood test that measures prostate-specific antigen, a protein made by normal and cancerous prostate cells. A level above 4.0 ng/mL is generally called abnormal, but BPH, infection and other causes also raise it. The number starts a conversation with your doctor. It is not a diagnosis.

What does a PSA test measure?

A PSA test measures prostate-specific antigen in your blood. PSA is a protein made by both normal and cancerous prostate cells (NCI, PSA Test).

A level above 4.0 ng/mL is generally considered abnormal. That cutoff is a guide, not a line between healthy and sick. Many things move the number up or down, so one reading says little by itself.

What can change a PSA result
FactorEffect on PSA
Enlarged prostate (BPH)Can raise it
Prostatitis or infectionCan raise it
Recent biopsyCan raise it
Vigorous exercise, ejaculationCan raise it
Finasteride, dutasterideLower it
Prostate cancerCan raise it

How accurate is a PSA test?

A PSA test is imperfect. It can flag a problem that is not cancer, and it can miss one.

About 6% to 7% of men get a false-positive result in a screening round. Only about 25% of men biopsied for a high PSA are found to have cancer (NCI, PSA Test). Screening may find cancer earlier, and a meta-analysis found a small drop in prostate cancer deaths over 10 years. Overdiagnosis can also lead to urinary, bowel and sexual side effects (NCI, PSA Test).

A number, not a verdict

A high PSA often leads to repeat tests first. Ask your doctor what the next step would be before you agree to screen.

Who should get a PSA test?

That is a personal decision made with your doctor. There is no single answer for every man.

The USPSTF advice, as described by the CDC, is that men ages 55 to 69 should decide for themselves after talking with a doctor. Men 70 and older should not be screened routinely, for men with no symptoms and no prior prostate cancer (CDC, Prostate Cancer Screening).

  • Ask about benefits and harms before testing.
  • Share your family history of prostate cancer.
  • Report urinary symptoms, since they need an exam whatever the PSA says.
  • Keep taking prescribed medicine, and tell the lab or doctor what you take.

Urgent signs are different. Blood in urine, fever or chills with painful urination, or severe belly, back or side pain need same-day care. If you cannot urinate at all, go to the emergency room.

Do supplements change PSA results?

Some can, so tell your doctor about every supplement before a test. Prostadine has not been tested for any effect on PSA, and it does not lower PSA.

NCCIH says saw palmetto does not appear to change PSA (NCCIH, Saw Palmetto). That is a statement about the herb in studies, not about Prostadine, whose amounts are not printed. Nothing is known about how the other eight ingredients affect the test.

The reverse risk matters more. Finasteride and dutasteride lower PSA, so your doctor needs to know if you take them (NCI, PSA Test). Ask your doctor or pharmacist about any new supplement. Our guide on supplements and prostate medicines lists what to bring, and the prostate basics guide explains the gland itself.

FAQ01

Quick answers on the PSA test

Do I need to fast before a PSA test?

Usually not, but follow your lab's instructions. Doctors often ask you to avoid vigorous exercise and ejaculation for a short time before the draw, since both can raise PSA (NCI, PSA Test). Ask the office what applies to you.

Is a PSA test the same as a digital rectal exam?

No. PSA is a blood test, and a rectal exam is a hands-on check of the gland. The USPSTF does not recommend digital rectal exams for screening (CDC, Prostate Cancer Screening), though a doctor may still use one to look into symptoms.

Can a normal PSA still mean a problem?

Yes. A normal result does not rule out cancer or explain urinary symptoms. New symptoms, a family history or a rising trend over time can matter more than one reading, so keep your follow-up visits.
