Saw Palmetto for BPH: The Large Trials in Plain Terms
Saw palmetto for BPH has not beaten placebo in the largest trials. A 2006 trial of 225 men and a 2011 trial of 369 men, including triple-dose arms, found no symptom benefit. A Cochrane review of 32 trials agreed. Saw palmetto does not treat BPH, and Prostadine lists it without any printed amount.

Does saw palmetto work for BPH?
No large trial has shown that saw palmetto works better than placebo for BPH urinary symptoms. NCCIH says it is probably not helpful for them (NCCIH, Saw Palmetto).
BPH means the prostate has grown larger with age. It affects about 5% to 6% of men aged 40 to 64 and 29% to 33% of men 65 and older, and it is not cancer (NIDDK, Enlarged Prostate (BPH)).
Saw palmetto is a palm berry extract. The trials below tested whether it helps.
| Study | Who | Result |
|---|---|---|
| Bent, NEJM 2006 | 225 men, 1 year | No better than placebo (Bent et al., NEJM 2006) |
| CAMUS, JAMA 2011 | 369 men, 72 weeks, up to triple dose | No better than placebo (Barry et al. (CAMUS), JAMA 2011) |
| Cochrane 2012 | 32 trials, 5,666 men | No benefit on symptoms, flow or size (Tacklind et al., Cochrane 2012) |
What did the large saw palmetto trials test?
They tested saw palmetto against a dummy pill in men with BPH symptoms. Neither group knew which pill it received.
The first trial ran one year. The CAMUS trial went longer and raised the dose in steps, up to three times the usual amount. Even at the top dose, symptoms did not improve more than with placebo (Barry et al. (CAMUS), JAMA 2011).
The Cochrane pooled result showed no gain on symptom score, night urination, urine flow or prostate size (Tacklind et al., Cochrane 2012).

Neither dose beat placebo for BPH symptoms.
Bars compare study doses or official limits with each other, not with Prostadine: its amounts are not printed, so its bar stays empty. No trial has tested Prostadine itself.
The bars show amounts used in studies and an empty bar for Prostadine, because its amount is not printed. Study amounts say nothing about what the bottle holds.
Can saw palmetto still help some men with BPH?
A few men may feel better, but placebo can explain that. In the Cochrane review, about 43% of men on double or triple doses had a response, against about 44% on placebo (Tacklind et al., Cochrane 2012).
These results cover saw palmetto alone, in studied extracts. A drop that blends nine ingredients is a different product. Nothing in the trials tests Prostadine.
The sales page lists saw palmetto with the line "Kidney supporting properties" and also "Strong antimicrobial". We found no study that backs either phrase for this product.
Saw palmetto does not appear to change PSA readings (NCCIH, Saw Palmetto). A PSA result still needs a doctor to read it.

See packsWhat should you do about BPH symptoms instead?
See a doctor for an exam, since weak stream, night trips and urgency have several causes. The NHS lists lifestyle steps and medicines such as tamsulosin, finasteride and dutasteride for bothersome cases (NHS, Prostate Enlargement).
- Cut back on alcohol and caffeine.
- Drink less in the evening.
- Keep a short diary of night trips for your visit.
- Read our prostate basics guide before the appointment.
Keep taking prescribed medicine. Cannot pass urine at all, blood in urine, fever or chills with painful urination, or severe belly, back or side pain means same-day care, and being unable to urinate means the emergency room.
See the saw palmetto page and the supplements guide for more.

FAQ01
Quick answers on saw palmetto and BPH
Why did earlier studies make saw palmetto look helpful?
Many early studies were short, small or lacked strict blinding. Placebo responses in urinary symptoms are large. When two larger trials used tighter designs, the benefit over placebo disappeared (Barry et al. (CAMUS), JAMA 2011).
Does saw palmetto shrink the prostate?
No benefit on prostate size appeared in the pooled Cochrane analysis of 5,666 men (Tacklind et al., Cochrane 2012). Saw palmetto does not shrink an enlarged prostate. Only a doctor can tell you which approach fits your size, symptoms and risks.
Is it worth trying saw palmetto for a few months anyway?
That is a personal choice with your doctor. Evidence says odds of benefit beyond placebo are low, and delay can hide a problem that needs care. If you try it, tell your doctor and keep your appointments.
